  • Inngangur: Stam ungra barna er mjög breytilegt. Sama barnið getur stamað mjög mikið einn daginn en lítið þann næsta. Fræðimenn hafa velt fyrir sér ástæðum breytileika í stami án þess að finna einhlítar skýringar. Mikilvægt er að skoða nákvæmlega breytileika stamsins með það í huga að hann kynni að varpa ljósi á eðli taltruflunarinnar. Einnig eru mælingar á þróun breytileika mikilvægar í greiningarvinnu svo hægt sé að taka ákvörðun um hvort þörf sé á meðferð og hvort búast megi við jákvæðum breytingum þegar hún hefst.
    Markmið: Skoðaður var breytileiki stams með því að halda breytum öðrum en tíma stöðugum. Með endurteknum mælingum á grunnskeiði var leitast eftir að skoða breytileika í stami þriggja leikskólabarna á rúmlega þriggja mánaða tímabili. Breytileiki var mældur með því að skoða breytingar í tíðni stamaðra atkvæða, hvernig hraði talsins breyttist og hvort breytingar áttu sér stað á tíu punkta alvarleikakvarða sem rannsakandi og foreldrar mátu á tímabilinu. Einnig var athugað hvort samræmi ríkti milli alvarleikamats sem rannsakandi gerði og þess mats sem foreldrar þátttakenda gerðu heimafyrir.
    Aðferð: Málsýni af frásögn og sjálfsprottnu tali þriggja barna á aldrinum 3;1-4;7 ára voru tekin upp á myndband. Tal hvers og eins var tekið upp átta sinnum á rúmlega þriggja mánaða tímabili og þannig var fylgst með öllum breytingum á talflæði þeirra. Aðstæður voru eins líkar og hægt var í hvert skipti. Sami viðmælandi hitti börnin, ræddi við þau á sama tíma dags, staðsetning viðtalsins var alltaf sú sama og verkefnin einnig.
    Niðurstöður: Í frásagnarhluta og sjálfsprottnu tali kom breytileiki fram í prósentu stamaðra atkvæða, í hraða tals og í alvarleikamati. Breytileikinn var þó mismikill eftir þátttakendum. Breytileiki kom oftast og mest fram í mælingum á prósentu stamaðra atkvæða en kom sjaldnast og minnst fram í alvarleikamati rannsakanda. Samræmi milli alvarleikamats rannsakenda og foreldra reyndist ekki mikið en aðstæður alvarleikamats þessara aðila voru ólíkar. Rannsakandi mat alvarleika eins og hann birtist í málsýnum sem tekin voru upp um miðjan daginn á meðan foreldrar byggðu alvarleikamat sitt á samskiptum sínum við börn sín frá morgni til kvölds.
    Ályktanir: Þar sem breytileiki í stami leikskólabarnanna kom fram í allnokkrum mælingum, þrátt fyrir að líklegum áhrifabreytum væri haldið stöðugum á rannsóknartímabilinu, má álykta að breytileiki hafi verið óstýranlegur. Niðurstöðurnar benda til þess að breytileiki ráðist ekki af áhrifum frá viðmælanda eða staðsetningu. Breytileiki var hins vegar meiri í frásagnarverkefnum, sem gefur til kynna að tegund verkefnis hafi áhrif á stam í tali barna á leikskólaaldri. Niðurstöðurnar gefa þá vísbendingu að breytileiki komi fram í stami leikskólabarna vegna þess að hann sé hluti af einkennum þess en ekki afleiðing utanaðkomandi þátta.

    Background: The stuttering of preschool children tends to vary. Stuttering can be highly noticeable one day and barely heard the next. Scholars have explored possible reasons for this variability without finding satisfactory explanations. It is possible that further studies on the variability in stuttering can explain the nature of the speech disorder. Moreover, it is important to monitor the development of the variability during assessment, in order to decide if a treatment is needed and if positive results can be expected, once treatment begins.
    Purpose: Over a period of three months the stuttering variability of three stuttering preschool children was examined. A multiple baseline design was used to explore if variability would occur in the stuttering of the children while possible influence factors were held constant. Variability was monitored by measuring the percent of stuttered syllables (SS%), changes in speech rate and assessing the stuttering severity on a ten point scale. Additional to the researcher´s measurements, the participant´s parents also rated the severity of their children´s stutter during the course of the study. Furthermore, the consistency between the severity ratings that the researcher and parents evaluated during the three months was compared.
    Method: Speech samples of narrative and spontaneous speech were video-recorded over a three month period. The participants were three children ages between 3;1- 4;7. The researcher met each child eight times over these months. All changes that occured in the participants speech flow were monitored. The research conditions were as similar as possible every time. The same interviewee met with the children and spoke to them at the same time during the day. The location of the interview was always the same, as well as the assignments the children were given.
    Results: Variability in the children´s stuttering was noticeable in both narrative and spontaneous speech. The percentage of stuttered syllables varied as well as the speech rate and the severity ratings but individual differences were prevalent. The variability was most obvious in measurements of stuttered syllables but emerged rarely in the severity ratings execuded by the researcher. There was little consistency in the severity ratings between the researcher and parents but it needs to be kept in mind that the evaluation procedures were different from one another. The researcher evaluated the severity as it appeared in the speech samples while the parents evaluated the severity of their children´s stuttering based on all encounters with them during the day.
    Conclusions: Since variability in the stuttering of preschool children emerged in considerable measurements, in spite of likely factors being kept stable during the research period, it can be concluded that the variability was incontrollable. The results suggest that the variability was not influenced by the interviewee or the locations. However, the variability was greater in all the narrative tasks, which implies that the type of tasks does influence the frequency and severity of stuttering. The results imply that variability emerges in the stuttering of preschool children because it is a symptom of stuttering and does not occur as a consequence of outside influences.
