  • Verkefnið er fræðileg samantekt rannsókna sem gerðar hafa verið á sambandi hreyfingar, þunglyndis og ónæmiskerfisins. Athugað verður hvort kemur á undan, þunglyndi eða ónæmisröskun og hvernig hreyfing hefur áhrif á sameiginlega undirliggjandi ferla raskananna verður. Mismunandi kenningar um tilurð þunglyndis verða skoðaðar: annars vegar kenningin um langvarandi virkjun streituviðbragða og hins vegar frumuboðakenningin um þunglynd. Það kemur í ljós að ónæmiskerfið er mikilvægt undirliggjandi stef í meingerð þunglyndis og mögulega lykillinn að áhrifum hreyfingar sem meðferð við sjúkdómnum. Spurningum varðandi áföll í sambandi við kviknun þunglyndis og virkjun ónæmiskerfisins verður velt upp og leitast við að svara þeim. Það orkar tvímælis að hreyfing, sem talin er fýsilegur meðferðarkostur við þunglyndi og góð leið til að styrkja ónæmiskerfið, hafi sambærileg áhrif á heilann og boðefnakerfi líkamans og sést í þessum röskunum. Skoðað verður hvort ónæmisfræðilegir þættir hafi forspárgildi varðandi svörun við meðferð þunglyndra og hvernig hreyfing hefur áhrif á þessa þætti. Aðeins fundust fjórar greinar sem skoðuðu öll þrjú efnin samtímis, sú elsta frá árinu 2006. Sambandið milli þunglyndis og ónæmiskerfisins og áhrif hreyfingar á það er ekki einfalt og rannsóknir eiga erfitt með að komast að sömu niðurstöðu. Höfundur dregur þá ályktun að hægt sé að fá þau jákvæðu áhrif sem cortisól á að hafa á heilann í gegnum hreyfingu með því að auka IL-6. Möguleikinn á einstaklingsbundinni meðferð sem ákvörðuð er útfrá skimun á magni frumuboðanna TNF-α og IL-1β er athugaður og gæti gefið markvissari og hraðari svörun við meðferð þunglyndra.

    This paper reviews the existing literature on the relationship between exercise, major depression and the immune system. Which comes first, major depression or immunological disturbances? In what way does exercise influence a common underlying process? Different theories on the causes of depression will be examined: the relationship between longstanding hyper activation of systematic stress reflexes and depression and the cytokine hypothesis of depression. It becomes clear that the immune system is not just a common theme in different theories of the pathology of depression; it is also a possible gateway to a more effective treatment of the disease. Questions concerning trauma in relations to the onset of depression and activation of the immune system will be addressed. Exercise is considered a valid treatment option for major depression and for improving immune function. However, the fact that exercise has a similar effect on the brain and the cytokine system as depression and immune disturbances is controversial. This paper will shed light on whether immunological factors can be utilized to predict treatment responses in a depressed population and how exercise influences these factors. Four articles reviewing all three concepts at the same time were identified, the oldest one from the year 2006. The author concludes that the effect exercise has on major depression is mediated through the anti-inflammatory role of cortisol by increasing IL-6. This opens up for the possibility of a personalized treatment based on the quantity of the cytokines TNF-α and IL-1β, which will pave the way for safer treatments with shorter response time.
