  • Meginmarkmið þessa verkefnis var að kynna og koma á eftirfylgni með heilsu og færni barna með Cerebral Palsy (CP) á Íslandi. CP-eftirfylgnikerfið er annars stigs forvörn ætluð til að fyrirbyggja síðbúna alvarlega fylgikvilla upprunalega CP- skaðans. Kerfið veitir möguleika á að fylgjast með færni einstaklinga með CP og meta mismunandi meðferðaríhlutanir. Með eftirfylgninni er markmiðið einnig að auka samvinnu fagstétta og bæta samræmingu þjónustu við börn og ungmenni með CP. Kerfisbundið mat, sem er hluti af eftirfylgninni, gerir kleift að fylgjast með faraldsfræði og heilsu allra einstaklinga með CP á Íslandi og auka þar með þekkingu á CP á Íslandi. Verkefnið var þríþætt: i. Þýðing á sænska eftirfylgnikerfinu CPUP, sem metur heilsu og færni barna/ungmenna með CP, yfir á íslensku sem CP eftirfylgni (CPEF). ii. Prófun á notagildi þverfaglegs mats, sem er hluti af eftirfylgninni, með því að skrá heilsu og færni 28 barna með CP samkvæmt CPEF-eftirfylgnikerfinu. iii. Samræming og þjálfun þverfaglegs samstarfs fagfólks. Endurgjöf var fengin frá foreldrum barna og fagfólki sem tók þátt í prófun á notagildi CPEF-matsins. Prófunin leiddi til þéttara eftirlits hjá sex börnum af 28 sem tóku þátt í verkefninu, mælt var með skurðaðgerð hjá einu barni og mælt var með reglulegu eftirliti barnalæknis fyrir öll börnin. Flestir foreldrar og fagmenn sem tóku þátt voru sammála um að samvinna myndi aukast meðal fagfólks og að þjónustan yrði heilsteyptari og fyrirsjáanlegri með samræmdri skoðun og mati. Í framtíðinni er mælst til þess að skoðun sem þessi verði framkvæmd á fyrirfram ákveðnum tímabilum og sameinuð öðrum íhlutunum sem barnið þarfnast eða verði framkvæmd í heimabyggð þess. Stöðlun og samhæfing meðferðar og eftirfylgni á landsvísu, getur stuðlað að bættri heilbrigðisþjónustu fyrir alla einstaklinga með CP á Íslandi, óháð búsetu og samfélagslegri stöðu. Með reglubundinni eftirfylgni er hægt að bæta lýðheilsu hópsins.

    Abstract
    The overall aim of this project was to present and start implementing a systematic follow-up healthcare programme for children with Cerebral Palsy (CP) in Iceland. The CP healthcare follow-up is a secondary prevention programme intended to prevent secondary complications of the CP impairment. Following this programme it will be possible to monitor the individual function and thereby evaluate different interventions. Other aims of the programme are to increase interdisciplinary teamwork among healthcare professionals and improve coordinated service for children and youth with CP. Systematic evaluation, as a part of this programme, will provide epidemiological information and enable us to describe the natural course and health condition of individuals with CP in Iceland. The project was divided into three phases: i. Translation of the Swedish healthcare programme, CPUP (Cerebral Palsy Uppföljning), into Icelandic as Cerebral palsy eftirfylgni, CPEF. ii. Testing the interdisciplinary assessment forms, by evaluating 28 children with CP according to the CPEF follow-up programme. iii. Coordinating the interdiciplinary teamwork of the participating healthcare workers. Participating parents and healthcare workers were asked to give feedback on the process and the follow-up programme by answering an anonymus questionnaire. The testing process resulted in a more intensive radiological follow-up of the hips for six of the participants, orthopaedic operation was recommended for one participant and a more systematic follow-up by a pediatrician was recommended for every participant. Almost every respondent to the questionaire agreed on that the follow-up programme can improve the interdiciplinary teamwork and by the coordinated evaluation the service will be more holistic and in the future more predictable. In the future it is recommended that the evaluation will be executed in predetermined intervals and combined with other interventions or it will be performed in the childs local environment. Implementing a nationwide follow-up healthcare programme may lead to (more) equal health service to all children and adolescents with CP in Iceland, regardless of their residence or social status. The public health can be improved by closer systematic follow-up of this population.
