  • Skjólstæðingsmiðuð þjónusta hefur verið skoðuð í auknu mæli undanfarin ár bæði frá sjónarhorni fagaðila og skjólstæðinga. Ýmsar hindranir hafa komið í ljós við framkvæmd þjónustunnar sem tengjast fagaðilum, skjólstæðingum, þjónustukerfi og samspili þar á milli. Forsendur þessarar rannsóknar er sú að skjólstæðingsmiðuð þjónusta auki ánægju skjólstæðinga og skili betri árangri í heilbrigðiskerfinu og byggir verkefnið á hugmyndafræðinni um skjólstæðingsmiðaða nálgun. Tilgangur rannsóknarinnar var að kanna upplifun einstaklinga með geðræn veikindi af þjónustu geðdeildar Sjúkrahússins á Akureyri (FSA) og að hve miklu leyti hún samræmdist grundvallarþáttum skjólstæðingsmiðaðrar þjónustu. Leitast var við að svara eftirfarandi rannsóknarspurningu: Hvernig samræmist upplifun skjólstæðinga, af þjónustu geðdeildar FSA, grundvallarþáttum skjólstæðingsmiðaðs starfs? Rannsóknin er eigindleg og vinnulag grundaðrar kenningar var notað við greiningu gagna. Gagnaöflun fór fram með viðtölum við sex fyrrverandi skjólstæðinga geðdeildar FSA á aldrinum 19 til 46 ára sem voru valdir með markmiðsúrtaki. Viðtalsramminn byggði á grundvallarhugtökum skjólstæðingsmiðaðs starfs. Niðurstöður bentu til þess að þátttakendur voru almennt ánægðir með þjónustu geðdeildar FSA og var viðmót og samvinna starfsfólks, val á viðfangsefnum og fræðsla helstu kostir deildarinnar. Þjónustan samræmdist að töluverðu leyti grundvallarþáttum skjólstæðingsmiðaðs starfs og það kom skýrt fram hvernig þjónustukerfið og starfsfólkið ýmist stuðlaði að eða hindraði skjólstæðingsmiðaða þjónustu. Það sem þarf einna helst að huga að er aðkoma þátttakenda í ákvörðunartöku og auka þarf fræðslu til aðstandenda svo að þeir geti verið virkir þátttakendur í þjónustuferlinu. Einnig þarf að skoða samskipti og upplýsingaflæði milli fagaðila til að tryggja samræmi og skýrari stefnu í þjónustunni.

    Client-centred practice has been studied more and more over the past years both from a professional and client point of view. When implementing this service, various barriers emerge involving interplay related to the professionals, clients and service systems. The assumption for this study is that client-centred practice increases client satisfaction and improves results in the health care system. The study is built on the ideology of the client-centred approach. The aim of the study was to explore the experiences of individuals with mental illness of services provided by the Psychiatric Unit of Akureyri Hospital (FSA) and to what extent it is in accordance with the core concepts of client-centred practice. The following research question was put forward: How well does the client’s perspective of the services offered by the Psychiatric Unit match the core concepts of client-centred practice? The study method is qualitative and analysis of data was based on the grounded theory approach. Data was collected through interviews with six former client of the Psychiatric Unit at FSA between the ages of 19 and 46, each client was chosen through the process of purposive sampling. The interview framework was built on the core concepts of client-centred practice. The results showed that the participants were generally satisfied with the Psychiatric Unit’s services. The attitude and co-operation of the staff, choice of activities and the participants education being the main positive points of the Unit. The service was largely in accordance with the core concepts of client-centred practice and it clearly indicates how the service system and the staff can either support or hinder client-centred practice. The main issues that need to be considered are the participant’s access to the decision making process and more education to family members in order for them to be active participants in the service process as well. Information flow between professionals also needs to be explored to ensure better coordination and clearer policies in the services.
