  • Fjöldi fyrirbura fer vaxandi í heiminum og á hverju ári hér á Íslandi standa ótal fjölskyldur frammi fyrir þeim erfiða veruleika að eignast fyrirbura eða veikan nýbura. Rannsóknir á upplifun foreldra af því að barnið þeirra leggst inn á nýburagjörgæslu sýna að það er þeim mikið áfall og þeir upplifa flóknar tilfinningar á borð við ótta, valdleysi, áhyggjur og depurð. Þegar foreldrar eru aðskildir frá nýfæddu barni sínu getur það leitt af sér erfiðar minningar sem geta lifað með þeim árum saman.
    Aðskilnaður móður og barns hefur verið rannsakaður mikið og sýna rannsóknir samhljóma niðurstöður um neikvæðar afleiðingar hans. Í kjölfarið hafa heyrst háværar raddir þar sem stofnanir eru hvattar til að styðja við stöðuga samveru móður og barns. Helsti ávinningur stöðugrar samveru eru sterkari tengsl milli móður og barns sem hefur þroskahvetjandi áhrif á barnið til frambúðar. Einnig styður samvera við árangursríka brjóstagjöf, með sínu þekktu kosti.
    Sængurlegan á Íslandi og annarsstaðar í heiminum er að styttast og færast í auknum mæli inn á heimili foreldra. Er einn kostur þess að fjölskyldan er saman sem ein heild og ljósmóðir sinnir henni sem slíkri. Þegar barn leggst inn á Vökudeild er slík samvera ekki kostur og þurfa því foreldrar þeirra barna aðra þjónustukosti.
    Í þessari fræðilegu úttekt var leitað eftir heimildum á viðurkenndum gagnagrunnum eftir rannsóknum sem fjalla um foreldra með barn á nýburagjörgæslu, bæði fyrirbura og veika nýbura. Var áhersla lögð á rannsóknir birtar árin 2004-2014. Reynt var að varpa ljósi á þarfir foreldra með barn á nýburagjörgæsludeild og þá sérstaklega foreldra með síðfyrirbura, það er börn fædd eftir 35,0 vikna meðgöngu til 36,6 vikur og viðkvæma nýbura. Skoðaðar voru meðal annars íslenskar rannsóknir á sængurlegunni og hvaða form hennar þykir ákjósanlegt og hvernig sé best að koma til móts við áðurnefndan foreldrahóp og hvar öruggt sé að sinna síðfyrirburum og viðkvæmum nýburum í ljósi viðkvæms ástands þeirra.
    Helstu niðurstöður voru að mæður með barn á nýburagjörgæslu þrá að vera nálægt barni sínu meira en nokkuð annað. Aðskilnaður var erfiðasta upplifun þeirra af því að barn þeirra var á nýburagjörgæslu. Þarfir síðfyrirbura og viðkvæmra nýbura eru misjafnar en meta þarf hvert barn fyrir sig og ávallt skal hvetja til samveru móður og barns og hugsanlega er samvera möguleg í meira mæli en nú er gert á Landspítalanum.
    Með nýju fyrirkomulagi í sængurlegu kvenna með barn á Vökudeild Landspítala skapast tækifæri til að taka skref í átt að aukinni samveru móður og barns með nýjum áherslum í umönnun sængurkvenna með barn á Vökudeild.

    The number of premature newborns is increasing in the world and in Iceland many families are coping with a premature baby or a sick newborn. Reaserch on parents’ emotions and experiences who have had their newborn admitted to a neonatal intensive care unit (NICU) suggest this to cause major trauma for the parents and they experience complex feelings of fear, powerlessness, worry and sadness. When parents are separated from their newborn it can create negative memories that can remain with them for years.
    The separation of mother and her infant has been studied for many decades and the conclusions are consistent in showing its negative consequences. This has led to an outcry for institutions that care for mothers and newborns to support and encourage rooming-in. One of the main benefits of rooming-in is stronger bonding between mother and her newborn, which supports better developmental growth for the newborn for life. Rooming-in has also been shown to support more successful breastfeeding with its known benefits.
    Postpartum care in Iceland and other parts of the world is decreasing in the hospital environment and increasing in the home of the parents. One of the main benefits of homecare is that the family can be together as a unit and a midwife can give the family unique individual family care. When a newborn is admitted to the NICU, this wholeness and togetherness is not possible and the parents need other options during the postpartum period.
    This literature review was based on studies found in recognized databanks, focusing on parents with a newborn baby in the NICU, both premature babies and sick newborn’s. Emphasis was on publications from the years 2004-2014. The review attempts to shed a light on parents´ needs when their newborn is admitted to the NICU, special focus was on late premature babies, born after 35,0 weeks pregnancy to 36,6 weeks, and vulnerable full term babies. Icelandic reasearch on the postpartum period were assessed and what form of postpartum care is considered the optimum care and what kind of care meets the needs of NICU parents and where it is considered safe to care for late preterms and vulnerable newborns with their delicate situation.
    The main findings in this review were that mothers with a newborn admitted to NICU whish above all to to be close to their babies. Seperation was the most difficult experience they had in regard to their babies stay at the NICU. The needs of late premature and vulnerable full term newborns cover a wide range and each newborn should be assessed individually. Rooming-in should always be practised when possible and potentially Landspítali can practice that in more extent.
    A new arrangement for the care of postpartum women whose newborn is admitted to the NICU in Landspítali University hospital creates an opportunity to increse rooming-in for these families and put new focus of care for postpartum women with newborns admitted to NICU.
