  • Hvítblæði er illkynja sjúkdómur í blóðmyndandi frumum líkamans sem einkennist af offjölgun hvítra blóðkorna í beinmerg af mismunandi þroskastigum eftir tegund hvítblæðis. Illkynja frumur hvítblæðis eru ýmist af mergfrumu eða eitilfrumu uppruna og getur krabbameinið annað hvort verið brátt eða langvinnt. Meðferðarúrræðum er ábótavant einkum fyrir fullorðna einstaklinga með bráða hvítblæði þar sem aðeins 30 – 50% sjúklinga læknast.
    Efnatogviðtakinn CXCR4 og bindill hans CXCL12 gegna mikilvægu hlutverki í nýmyndun og viðhaldi blóðfruma. Við bindingu CXCR4 við CXCL12 færast óþroskaðar frumur í nærumhverfi beinmergsins þar sem þær fá að vaxa í friði. Í sjúkdómsástandi eins og hvítblæði gefur þessi binding illkynja frumum vernd frá frumudrepandi krabbameinslyfjum. Plerixafor er fyrsta lyfið sem hindrar bindingu CXCL12 við CXCR4 sem samþykkt var á markaðnum. Í fyrstu var lyfið notað í meðferð gegn HIV en í dag er lyfið aðallega notað fyrir stofnfrumusöfnun en lyfið ýtir stofnfrumum úr beinmergnum og út í blóðrásina þar sem þeim er safnað. Nýjustu notkunarmöguleikar lyfsins eru að losa hvítblæðisfrumur úr nærumhverfi beinmergsins út í blóðrásina þar sem krabbameinslyf ná til þeirra og drepa.
    Markmið þessarar rannsóknar var að skoða hvaða áhrif efnatoghemillinn Plerixafor hefur á hvítblæðisfrumur með því að skoða samspil lyfsins við önnur krabbameinslyf, athuga hvort lyfið auki stýrðan frumudauða, athuga hvort það hafi áhrif á útþroskun AML fruma og skoða áhrif þess á frumuskrið AML fruma að CXCL12.
    Í forrannsókn verkefnisins var hannað frumuræktunarmódel sem nýtist vel til lyfjanæmisprófs í AML. Stýrður frumudauði var mældur með flúorlitunum Annexin V FITC og Propidium iodide (PI) í frumuflæðisjá. Til að rannsaka útþroskun fruma var sett upp kólóníuræktun og auk þess sem litað var með mótefnalitum fyrir sameindum sem einkenna þroskaðar frumur. Frumuskriðstilraun var sett upp í hefðbundnum frumuskriðsbökkum.
    Rannsóknin sýnir fram á að hægt sé að skoða áhrif mismunandi styrkleika krabbameinslyfja á hvítblæðisfrumur sjúklinga í frumuræktunarmódeli og meta næmi illkynja fruma gegn krabbameinslyfjum. Rannsóknin sýnir fram á tilhneigingu Plerixafor til að auka frumudauða með þekktum krabbameinslyfjum í frumuræktunarmódelinu en áhrif Plerixafor til að auka frumudauða eru skýr í kólóníuræktunum. Ekki er sýnt fram á að Plerixafor auki útþroskun fruma í kólóníuræktun né með breytingum á tjáningu yfirborðssameinda. Sýnt er fram á hæfni Plerixafor til að hindra frumuskrið í átt að CXCL12.
    Niðurstöðurnar gefa því vísbendingu um frumudrepandi áhrif Plerixafor á hvítblæðisfrumur til viðbótar við tilfærslu þeirra út í blóðrásina en rannsóknir eiga eftir að leiða í ljós hvort meðferð með Plerixafor samhliða hefðbundinni krabbameinslyfjameðferð muni bæta meðferðarárangur í bráða hvítblæði.

    Leukemia is a malignant disease of the hematopoietic stem cells, characterized by overgrowth of white blood cells in the bone marrow of a different stage maturation. Malignant cells are either of myeloid or lymphoid origin and the leukemia can be either acute or chronic. Treatment results are still insufficient, especially for adults with acute leukemia in which only 30-50 % of patients are cured.
    The chemokine receptor CXCR4 and its ligand CXCL12 play an important role in the proliferation and maintenance of blood cells. When CXCL12 binds to CXCR4 it causes chemotaxis of immature blood cells to the bone marrow microenvironment, where they have peace to grow. In leukemia, this interaction causes chemotaxis of malignant cells to the bone marrow microenviroment where they are protected from chemotherapy. Plerixafor is the first drug aproved by the Food and Drug administration, which inhibits the binding of CXCL12 and CXCR4. At first, the drug was used in treatment of HIV, but today the drug is mainly used for stem cell mobilization in lymphoma and myeloma as it causes the stem cells to leave the bone marrow and go into the bloodstream where the cells are collected. The latest ideas for the drug is to mobilize the leukemia cells from the bone marrow microenviroment into the bloodstream where chemotherapeutic agents can reach them and induce apoptosis.
    The aim of this study was to examine the effects of the chemotaxis inhibitor Plerixafor on leukemia cells by examining the interaction of the drug with other chemotherapeutic agents, investigate if the drug increases apoptosis, to see if it increases maturation of AML cells and see what effects it has on chemotaxis of AML cells to CXCL12 .
    In the pilot study we designed a cell culture model that is useful to test drug sensitivity and the effects of Plerixafor on AML cells. Apoptosis was measured by Annexin V FITC and Propidium iodide (PI) by flow cytometry. Colony forming assay was used to determine increased maturation of AML cells and in addition maturation was examined by flow cytometry. For the chemotaxis studies we used conventional chemotaxis wells.
    The study demonstrates that it is possible to examine the effect of different concentrations of chemotherapeutic agents in leukemia cells of patients with the cell culture model and evaluate the sensitivity of malignant cells against chemotherapy. The study demonstrates that Plerixafor can potentially increase cell death with known chemotherapeutic agents in the cell culture model, but the ability of Plerixafor to induce cell death in colony forming assays was confirmed. Plerixafor did not increase maturation of AML cells in colony forming assays nor by changes in expression of surface molecules. The study did demonstrate the ability of Plerixafor to inhibit chemotaxis towards CXCL12.
    The results of this study give an indication of incresed cell death with Plerixafor in addition to the known effect on leukemia cell mobilization. Future studies will have to show if Plerixafor combined with conventional chemotherapy will improve outcome in acute myelogenous leukemia.
