  • Börn sem upplifa verki og ótta í tengslum við inngrip eða meðferðir í æsku geta orðið fyrir neikvæðri upplifun sem getur fylgt þeim inn í framtíðina. Ein leið til að draga úr þessum afleiðingum gæti verið með hugardreifingu, t.d. með því að horfa á efni í spjaldtölvu. Hugardreifing í þessu skyni er þó lítið rannsökuð.
    Rannsókn þessi er forprófun og tilgangur hennar var að meta hugsanleg áhrif spjaldtölvu sem hugardreifingar á verki og ótta barna við ástungu í lyfjabrunn (port-a-cath®).
    Rannsóknarsniðið var hálfstöðluð tilraun með upphafs- og eftirsniði. Ótti og verkir voru metnir án spjaldtölvu í fyrra skiptið sem börnin fengu nál í lyfjabrunninn en með spjaldtölvu í seinna skiptið. Reynsla þátttakenda var einnig skoðuð og hvort hún geti haft áhrif á upplifun þeirra. Þátttakendur voru 14 börn á aldrinum 20 mánaða til 16 ára, 9 drengir og 5 stúlkur. Börnin mátu verki eftir inngrip og ótta fyrir ástungu með NRS (1-10) eða andlitskvörðum í öllum tilfellum nema þremur en þá mat móðir verki með NRS eða FLACC verkjamatskvarðanum. Notuð var lýsandi tölfræði og t-próf.
    Shapiro-Wilk próf sýndi marktæka dreifingu (p<0,05) á verkjum og ótta en þó var meirihluti barna (n=8) óttalaus fyrir inngrip. Meðaltal verkja var 2,90 (s=3,67) og meðaltal ótta 3,67 (s=3,76). Niðurstöður sýndu ekki marktækan mun á verkjum eftir magni ótta fyrir inngrip en p-gildið mældist 0,09. Þau börn sem mældust með ótta (n=5) fyrir ástungu mældust með marktækt minni verki þegar spjaldtölva var notuð (p<0,05). Hvorki var munur á ótta og verkjum eftir kyni né aldri. Niðurstöður sýna ekki mun á verkjum eftir meðferðaraðferðum (p=0,20). Börnin höfðu öll töluverða reynslu af ástungu í lyfjabrunn og höfðu nokkur þeirra þróað með sér aðferð til að takast á við inngripið eða fengið aðstoð til þess.
    Tilefni er til frekari rannsókna með stærra úrtaki og skoða áhrif reynslu á árangur meðferðarinnar.

    Children who experience pain and anxiety while undergoing interventions or treatments during hospitalization at a young age can experience negative feelings which can influence how they experience the healthcare assistance they receive in the future. The use of distraction to reduce the amount of pain and anxiety that children perceive during an intervention has received little attention from researchers.
    This research is a pretest and its purpose was to evaluate the use of a tablet computer as a source of distraction from pain and fear when children undergo needle insertion in a port catheter (port-a-cath®).
    The research design was quasi-experimental with pretest-posttest design. Pain and fear were first evaluated without the distraction of a tablet computer. The second time pain and fear were evaluated while a tablet computer was used as a source of distraction. The childrens prior experience of intervention and treatment was explored to see if it could have any influence on the level of their pain and fear. The sample size was 14 children from the age 20 months to 16 years, 9 boys and 5 girls. The children evaluated their pain and fear with Numeric Rating Scale (NRS) and faces scales in all cases except three. In those three cases the mothers evaluated the children´s pain and fear with the NRS or the Faces, Legs, Activity, Cry, Consolability scale (FLACC). Descriptive statistics and t-tests were used for data analysis.
    The Shapiro-Wilk test showed a significant distribution (p<0,05) for pain and fear but most of the children did not feel any fear before the intervention. The mean score for pain was 2,90 (s=3,67) and the mean score for fear was 3,67 (s=3,76). The results did not show a significant difference between pain and the quantity of fear before the intervention but the p-value was 0,09. Children who felt fear before the intervention (n=5) had significantly lower pain when a tablet computer was used (p<0,05). No difference was found between pain and fear regarding age and gender. The results did not show any difference in pain regarding the type of distraction (p=0,20). All of the children had substantial prior experience with needle insertion before the intervention and some of them had developed their own approach to deal with the intervention.
    More extensive reseach is needed in this area. There is also a need for further research with a larger sample size to explore the effect of children´s prior experience of intervention on treatment success.
