    Background: Adenine phosporibosyltransferase (APRT) deficiency is a metabolic disorder which is the cause of adenine oxidation to 2,8-dihydroxyadenine (DHA) instead of normal conversion to AMP. DHA is poorly soluble at physical level and therefore precipitates in urine and can cause of acute renal failure. The symptoms have been treated with allopurinol which converts to its active metabolite oxypurinol quickly after it enters the body. It is important to being able to quantify both oxypurinol and DHA in plasma for better diagnosis and treatment.
    Objective: Develop a quantification method with Ultra Performance Liquid Chromatography with tandem mass spectrometry (UPLC-MS/MS). In addition to sample preparation method development for DHA and oxypurinol quantification in plasma.
    Methods: Design of experiment (DOE) methodology was implemented to optimize UPLC-MS/MS assay for quantification of oxypurinol. Sample preparations methods with solid phase extraction (SPE) and protein precipitation (PPT) were developed for sample cleanup of plasma samples prior to quantification of DHA and oxypurinol.
    Results: UPLC-MS/MS assay was developed and used for quantifying oxypurinol in plasma samples from patients suffering from APRT deficiency. Sample preparation methods for cleanup of plasma samples for quantification of DHA and oxypurinol. Protein precipitation produced the best results for both compounds.
    Conclusions: Event though the optimized UPLC-MS/MS assay was used for quantifying oxypurinol in patient samples from patients suffering from APRT deficiency, the method needs validating for more accurate quantification. The recovery rate for DHA and oxypurinol must be determined for better quantification.

    efnaskiptasjúkdómur sem veldur því að í staðinn fyrir að adenín umbreytist í AMP með hjálp APRT þá oxast það yfir í 2,8-dýhýdroxýadenín (DHA). DHA er torleysanlegt í líkamanum og fellur því út í þvagi og veldur nýrnaskemmdum. Sjúkdómurinn hefur verið meðhöndlaður með lyfinu allopurinol sem umbreytist yfir í sitt virka form, oxypurinol, fljótlega eftir inntöku. Mikilvægt er að geta mælt bæði oxypurinol og DHA í plasma til að geta greint árangur lyfjameðferðarinnar.
    Markmið: Þróa magngreiningaraðferð fyrir oxypurinol með Ultra Performance Liquid Chromatography með raðtengdum massagreini (UPLC-MS/MS). Auk þess að þróa sýnameðhöndlunaraðferð fyrir plasma fyrir magngreiningu á DHA og oxypurinol.
    Aðferðir: Aðferðafræðinni Hönnun tilraunar var beitt til að hámarka UPLC-MS/MS aðferð fyrir magngreiningu á oxypurinol. Fastfasaskilun og próteinfelling voru sýnameðhöndlunaraðferðir sem voru þróaðar áfram fyrir hreinsun plasma sýna fyrir magngreiningu á DHA og oxypurinoli.
    Niðurstöður: UPLC-MS/MS aðferð var hámörkuð og notuð til að magngreina
    oxypurinol í plasmasýnum frá sjúklingum sem þjást af APRT skorti. Einnig voru þróaðar sýnameðhöndlunaraðferðir til að hreinsa plasma sýni fyrir DHA og oxypurinol magngreiningu en próteinfelling kom best út fyrir bæði efni.
