  • Inngangur: Heilsutengd lífsgæði einstaklinga sem fengið hafa heilablóðfall með eða án málstols eru víðast hvar í vestrænum samfélögum lítið eða ekki metin á fullnægjandi hátt með kerfisbundnum, réttmætum og áreiðanlegum hætti, þrátt fyrir að grundvallarmarkmið með hvers kyns inngripi sé að auka heilsutengd lífsgæði skjólstæðinga með beinum eða óbeinum hætti. Ein af ástæðum þess er skortur á mælitækjum. Til að sníða megi skilvirk meðferðarúrræði í heilbrigðisþjónustu er nauðsynlegt að meta áhrif heilsufarsástands á lífsgæði skjólstæðinga fyrir og eftir meðferð, sér í lagi þegar um er að ræða einstaklinga með málstol í kjölfar heilablóðfalls, en þeir búa við slök heilsutengd lífsgæði samanborið við flesta aðra sjúklingahópa.
    Markmið: Annars vegar að þýða og staðfærða The Stroke and Aphasia Quality of Life Scale-39g (SAQOL-39g) á íslensku, og hins vegar að forprófa þýðinguna til að kanna hvort mælitækið sé 1) aðgengilegt þátttakendum, 2) hvort innri áreiðanleiki þess sé fullnægjandi og 3) hvort endurtektarprófunaráreiðanleiki þess sé fullnægjandi. Að auki verða teknar saman niðurstöður þátttakenda úr forprófun.
    Aðferðafræði: Þýðing á SAQOL-39g var útfærð samkvæmt viðurkenndri bakþýðingaraðferð. Mælitækið var forprófað á 20 þátttakendum; tíu með málstol og tíu án málstols í kjölfar heilablóðfalls. Aðgengileiki var metinn með hlutfalli ósvaraðra prófatriða (<10%) og gólf- og rjáfuráhrifum (<80%), innri áreiðanleiki var metinn með Cronbach's alpha fyrir heildarútkomu og undirpróf (>0,7) og fylgni prófatriða við heildarútkomu (>0,3), og endurtektarprófunaráreiðanleiki var metinn með Intraclass Correlation Coefficient (ICC) fyrir heildarútkomu og undirpróf (>0,75). Endurtektarprófun fór fram innan 14 daga frá upphaflegri prófun.
    Niðurstöður: Bakþýðing leiddi til þess að átta prófatriði voru tekin til sérstakrar skoðunar fyrir lokaþýðingu. Engin prófatriði þurfti að staðfæra. Forprófun sýndi fram á fullnægjandi aðgengileika þar sem hlutfall ósvaraðra prófatriða var 0% og gólf- og rjáfuráhrif voru innan viðmiðunarmarka fyrir stök prófatriði og undirpróf (<80%). Innri áreiðanleiki var fullnægjandi fyrir heildarútkomu (α=0,94) og undirpróf; líkamlegt undirpróf (α=0,93), sálfélagslegt undirpróf (α=0,93) og samskiptafærni undirpróf (α=0,89). Fylgni prófatriða við heildarútkomu var á bilinu 0,30-0,82. Endurtektarprófunaráreiðanleiki var fullnægjandi fyrir heildarútkomu (0,95) og undirpróf; líkamlegt undirpróf (0,94), sálfélagslegt undirpróf (0,95) og samskiptafærni undirpróf (0,95). Meðalstigafjöldi þátttakenda var 3,96 stig (sf=0,62).
    Ályktanir: Próffræðilegir eiginleikar íslenskrar þýðingar SAQOL-39g eru fullnægjandi og standast samanburð við próffræðilega eiginleika frumútgáfu mælitækisins. Líklegt þykir að þýðingin búi yfir sambærilegu réttmæti og frumútgáfa. Niðurstöður úr forprófun benda til þess að heilsutengd lífsgæði einstaklinga með málstol séu marktækt lakari en einstaklinga án málstols í kjölfar heilablóðfalls.

    Background: Health-related quality of life of people who have suffered a stroke with or without aphasia are little or not systematically, validly and reliably measured in western societies, despite the fact that the fundamental purpose of any intervention is to improve the health-related quality of life of patients by direct or indirect means. This is partly explained by a lack of proper measurement tools. In order to construct efficient interventions in health care it´s necessary to measure the effects of a health condition on the quality of life of patients before and after intervention, especially when referring to patients with aphasia subsequent to stroke because their health-related quality of life have been shown to be worse than the health-related quality of life of most other groups of patients.
    Purpose: Firstly to translate and adapt The Stroke and Aphasia Quality of Life Scale-39g (SAQOL-39g) to icelandic, and secondly to test the psychometric properties of the icelandic SAQOL-39g, that is the 1) acceptability, 2) internal consistency and 3) test-retest reliability. Participants‘ test results will also be reviewed.
    Method: A traditional backtranslation method was used to translate the SAQOL-39g to icelandic. 20 participants who had suffered a stroke participated in the testing, there of ten with aphasia and ten without aphasia. Acceptability was based on missing data (<10%) and floor- and ceiling effects (<80%), internal consistency was measured by Cronbach‘s α for overall score and subdomains (>0,7) and correlation between test items and overall score (>0,3), and test-retest reliability was measured by Intraclass Correlation Coefficient (ICC) for overall score and subdomains (>0,75). Retesting was conducted within 14 days of original testing.
    Results: The backtranslation led to the reviewing of eight test items for the final translation. There were no test items that needed adaptation. The acceptability of the translation met criteria for both proportion of missing data (0%) and floor- and ceiling effects for test items and subdomains (<80%). Internal consistency also met ciriteria, both for overall score (α=0,94) and subdomains; physical domain (α=0,93), psychosocial domain (α=0,93) and communication domain (α=0,89). Correlation between test items and overall score ranged from 0,30 to 0,82. Test-retest reliability met criteria both for overall score (0,95) and subdomains; physical domain (0,94), psychosocial domain (0,95) and communication domain (0,95). The mean test score for participants was 3,96 points (sd=0,62).
    Conclusion: The Icelandic SAQOL-39g demonstrated good psychometric properties and is comparable to the original questionnaire. It is considered very likely that the translation has comparable validity to the original questionnaire. Testing results show preliminary evidence that the health related quality of life of people with aphasia are significantly worse that those of people without aphasia subsequent of stroke.
