  • Rannsóknir hafa sýnt fram á að heilsutengd lífsgæði fólks í skilunarmeðferðum eru skert. Á síðari árum hefur verið greint frá samspili einkenna líkt og síþreytu, verkjum og andlegri vanlíðan við heilsutengd lífsgæði. Mikilvægt er fyrir hjúkrun þessara sjúklinga að greina hvernig lífsgæði sjúklinganna koma fram í lokastigsnýrnabilun og hvort lífsgæði hafa áhrif á einkenni.
    Tilgangur þessa lokaverkefnis er tvíþættur. Í fyrsta lagi verður lýst heilsutengdum lífsgæðum sjúklinga sem fá mismunandi meðferð við nýrnabilun á lokastigi og í öðru lagi er ætlunin að skoða hvort tengsl eru milli lífsgæða og einkenna.
    Aðferð: Aðferðin sem varð fyrir valinu var samþætt fræðilegt yfirlit. Bæði megindlegar og eigindlegar rannsóknir voru valdar til að svara tveimur rannsóknarspurningum: 1) Hver eru heilsutengd lífsgæði nýrnasjúklinga sem fá meðferð við lokastigsnýrnabilun? og 2) Hver eru tengsl einkenna og heilsutengdra lífsgæða?
    Niðurstöður: Lífsgæði sjúklinga sem fara í blóðskilun eru skert m.t.t. almennrar heilsu, félagslífs og breytinga á daglegum venjum. Kviðskilunarsjúklingar virðast vera með betri andlegri líðan heldur en blóðskilunarsjúklingar. Nýraþegar meta lífsgæði sín betri en sjúklingar í blóð- og kviðskilun hvað varðar líkamlega virkni, áhrif nýrnasjúkdóms, byrði nýrnasjúkdóms, atvinnuþátttöku, félagsleg tengsl og félagslegt umhverfi, svefn og almenna heilsu.
    Rannsóknir sýna að algeng einkenni sem sjúklingar í skilunarmeðferðum hafa eru síþreyta, verkir og andleg vanlíðan. Verkir af ólíkum toga og síþreyta eru algeng en sjúklingar upplifa þessi einkenni einnig sem mjög óþægileg. Einkenni hjá sjúklingum í skilunarmeðferðum hafa verið flokkuð í einkennaklasa, einkenni innan ákveðins klasa birtast samhliða, tengjast hvort öðru og geta aukið áhrif hvors annars.
    Ályktun: Heilsutengd lífsgæði eru mismunandi milli ólíkra meðferðaforma við lokastigsnýrnabilun. Það virðist gagnlegt að meta einkenni heildstætt með tilliti til samspils einkennaklasa og hvernig sjúklingurinn upplifir einkenni sín. Mikil byrði af einkennum virðist draga úr heilsutengdum lífsgæðum. Mikilvægt er fyrir hjúkrunarfræðinga að þekkja samspil lífsgæða og einkenna hjá þessum sjúklingahópi.

    Studies have shown that health-related quality of life in dialysis is impaired. In recent years, there have been reports of interaction of symptoms like fatigue, pain and distress to the health-related quality of life. It is important for the care of these patients to analyze how the health-related quality of life of the patients appear in the various treatments for ESRD and whether the health-related quality of life affects symptoms.
    The purpose of this master thesis project is twofold. First, the aim is to describe the health-related quality of life of patients being treated with end stage renal disease and secondly, the intention is to look at how the relationship of symptoms and quality of life appear in research literature.
    Method: The method which was chosen was integrative review. Both qualitative and quantitative studies were selected to answer the following research questions: 1) What is the health-related quality of life of kidney patients treated for ESRD? and 2) What is the relationship between symptoms and health-related quality of life?
    Results: The health-related quality of life for hemodialysis patients are impaired in general health, social-life and lifestyle changes. Peritoneal dialysis patients appear to have a better psychosocial health than dialysis patients. When assessing their quality of life, kidney-transplant patients rate their health-related quality of life higher than patients in hemodialysis and peritoneal dialysis in the categories: physical activity, effects of kidney disease, renal disease burden, employment, social relations and social environment, sleep and general health.
    Studies show that common symptoms that patients in dialysis therapies have are fatigue, pain and mental distress. Pain of various types and fatigue are common, but patients experience these symptoms also as very unpleasant. Symptoms in patients with dialysis treatments have been grouped in clusters of symptoms, symptoms appear within certain clusters in parallel, related to each other and can enhance each other's effects.
    Conclusion: Quality of life is different between different forms of treatment for ESRD. It could be useful to assess the symptoms with regard to the interaction of clusters of symptoms and how the patient experiences symptoms. High burden of symptoms appears to reduce health-related quality of life. It is important for nurses to know the interaction between quality of life and symptoms in these patients.
