    Tilgangur þessa verkefnis var tvíþættur. Annars vegar að fræðast um mikilvæga og algenga þætti varðandi andlega vanlíðan kvenna á meðgöngu og draga fram mikilvægi geðheilsuverndar á meðgöngu. Hins vegar að taka viðtöl við ljósmæður í mæðravernd Heilsugæslu höfuðborgarsvæðisins til þess að fá innsýn í nálgun þeirra, vinnulag og viðhorf til geðheilsuverndar í mæðraverndinni. Notast var við fræðilega úttekt í fyrri hluta verkefnis og eigindleg viðtöl í seinni hluta þess.
    Tekin voru viðtöl við tvær ljósmæður í mæðraverndinni. Viðtölin voru tekin upp og skrifuð niður orð fyrir orð. Tekin voru út helstu atriði viðtalanna og fundust sex þemu sem voru eftirfarandi: Sjálfsagður hluti af mæðravernd, leiðbeiningar hjálpa, að hlusta og lækningamáttur samtalsins, tímaþröng og bið eftir sérfræðihjálp, að komast að orsök og vinna út frá því og að lokum þjálfun í samtalstækni og að þora að spyrja.
    Niðurstöður þessa verkefnis leiddu í ljós alvarleika andlegrar vanlíðunnar á meðgöngu. Að meta andlega líðan á meðgöngu er ekki síður mikilvægt en að meta líkamlega líðan. Andleg vanlíðan á meðgöngu getur haft alvarleg áhrif á heilsu móður og barns og haft neikvæð áhrif á fjölskylduna sem heild. Einnig kom í ljós að þær ljósmæður sem rætt var við höfðu jákvætt viðhorf til geðheilsuverndar, þær upplifðu að hafa úrræði og getu til þess að meta andlega líðan kvenna á meðgöngu en þó fannst þeim að hægt væri að gera betur. Ljósmæðurnar töldu að klínískar leiðbeiningar og vinnuleiðbeingar sem kynntar voru árið 2012, fræðsla, námskeið og vitundarvakning hjálpuðu ljósmæðrum við að finna og sinna þessum konum. Helstu hindranir í geðheilsuvernd voru bið eftir sálfræðihjálp og tímaskortur. Þar að auki fannst þeim mikilvægt að ljósmæður hefðu vald á góðri samtalstækni ásamt því að vera óhræddar við að spyrja um geðheilsu.

  • The purpose of this study was twofold: first, to learn about the importance and common aspects of mental distress for women during pregnancy, also to reveal the importance of mental care during pregnancy; second, to conduct interviews with midwives in antenatal care, to gain insight into their approach, procedures and attitudes toward mental health care.
    A literature review was used for the first part of the study and qualitative interviews for the second part. Interviews were conducted with two midwives from a maternity centers. The interviews were recorded then subsequently transcribed word for word. The text was analyzed and the six themes were identified: natural part of antenatal care; instructions help; listening and the healing of conversation; time pressure and wait for expert help; to get to the cause and work from there; communication skill training and the courage to ask.
    The results of this study show how severe the affects mental illness during pregnancy can have. The importance to assess mental health during pregnancy is no less than the importance to assess physical health. Additionally mental distress during pregnancy can seriously affect the health of the mother-and-child and adversely affect the family as a whole.
    It was also revealed that the midwives that were interviewed had a positive attitude towards mental care, they felt that they had resources and capacity to assess the mental health of women during pregnancy, but also felt that they could provide better care. The midwives thought that the clinical guidelines introduced in 2012, education, seminars, and awareness, helped midwives to identify and care for this group of women. Still they found that the main barriers to mental care were waiting for psychiatric help and lack of time. Moreover, they found it of importance that midwives have good communication skills as well as being unafraid to ask about mental health.
