    Ópíóíð verkjalyf eru öflugustu og algengustu lyfin sem eru notuð við meðal svæsnum eða slæmum verkjum. Hægt er að flokka ópíóíð lyf eftir ónæmisbælandi verkun þeirra. Morfín, kódein og fentanýl teljast til þeirra ópíóíð lyfja sem hafa sterk ónæmisbælandi áhrif, en tramadól, búprenorfín og oxýkódon teljast til þeirra ópíóíð lyfja sem hafa veik ónæmisbælandi áhrif. Síðastliðin ár hefur ópíóíð lyfjanotkun á Íslandi, sem og annar staðar í heiminum, aukist mikið. Vegna þessarar auknu notkunar ópíóíð lyfja hafa risið áhyggjur um langtíma aukaverkanir þeirra sem geta stafað frá röskun á innkirtla- og ónæmiskerfinu, og hugsanleg áhrif þess á þróun krabbameins.
    Markmið rannsóknarinnar var að kanna hvort ópíóíð lyfjanotkun hafi áhrif á nýgengi krabbameina og hvort munur sé á ópíóíð lyfjum sem hafa sterk og veik ónæmisbælandi áhrif.
    Gögn um brjósta-, blöðruhálskirtils-, ristils-, húð-, heila og miðtaugakerfis-, nýrna-, og magakrabbameins tilfelli sem greindust yfir tímabilið 2006-2013 voru fengin úr Krabbameinsskrá og notuð í rannsókn. Alls voru 3.401 tilfelli sem greindust á tímabilinu með sitt fyrsta krabbamein, þar af 1.728 karlar og 1.673 konur. Viðmið voru valin úr þjóðskrá fyrir hvert tilfelli og pöruð eftir aldri, kyni og búsetu. Alls fengust 27.546 viðmið, 13.379 karla og 14.167 konur. Gögn úr lyfjagagnagrunni Embættis Landlæknis um útleyst magn ópíóíð lyfja meðal tilfella og viðmiða voru fengin yfir tímabilið 2002-2013. Lyfjanotkun var skoðuð með tilliti til hvort einstaklingur fékk krabbamein eða ekki.
    Karlmenn sem fengu krabbamein voru líklegri til þess að hafa tekið ópíóíð lyf samanborið við viðmið sem höfðu ekki fengið krabbamein (líkindahlutfall (OR) 1,137,
    95% öryggisbil (CI) 1,022-1,265), slíkt samband var ekki að sjá meðal kvenna (OR 1,007, 95% CI 0,900-1,126). Neikvætt samband fannst milli aukinnar notkunar á ópíóíð lyfjum fyrir greiningu á fyrsta krabbameini meðal karlkyns tilfella í samanburði við viðmið sem voru án sögu um krabbamein, slíkt samband var ekki að sjá meðal kvenna.
    Ályktanir: Jákvætt samband virðist vera milli töku ópíóíð lyfja og nýgengis krabbameina meðal karla en ekki meðal kvenna. Aftur á móti, þegar greining er einskorðuð við karlmenn sem taka ópíóíð lyf, virðist neikvætt samband vera milli aukinnar notkunar og krabbameins. Þessar niðurstöður benda ekki til þess að samband sé á milli ópíóíð lyfjanotkunar og krabbameins hjá konum. Þessar fyrstu niðurstöður eru nokkuð mótsagnakenndar og þarfnast frekari athugunar.

  • Opioids are the most effective and commonly used analgesics for medium to severe pain. Opioids can be classified as strong or weak immunosuppressants. Morphine, codeine and fentanyl, are e.g. classified as strong immunosuppressants and tramadol, buprenorphine and oxycodone as weak immunosuppressants.
    The use of prescription opioid analgesics has been increasing in Iceland and in other countries in recent years and therefore it is important to examine whether the opioids immunosuppressive effects can potentially modulate cancer incidence among users of opioids.
    The aim of this study was to investigate whether opioid use affects cancer incident and the possible difference in immunosuppressive effects of opioids on cancer.
    Incident cases of breast- , prostate-, colon-, skin-, brain and central nervous system-, renal-, and stomach cancer diagnosed over the perioid of 2006-2013 were obtained via the Cancer register of Iceland. 3401 cases were included in the study with their first cancer diagnosis, of which there were 1728 men and 1673 women. The control group consisted of 27546 people of which, 13379 were men and 14167 women. Controls were obtained from the National Register of Iceland, all matched on age, gender and residence. Data from the Icelandic Directorate of Health on opioid use among cases and controls over the peroiod of 2002-2013 were obtained and used in this study. Opioid use was examined in relation to cancer incidence.
    Men diagnosed with cancer were more likely to have taken opioids compared to controls which did not have cancer (Odds ratio (OR) 1.137; 95% confidence interval (CI) 1.022-1.265), there was no such association among women (OR 1.007; 95% CI 0.900-1.126). When analysis excluded those who did not take any opioids a negative association was found for increased opioid use before first cancer diagnosis among male cases compared to male controls which did not have a history of cancer, there was no such association seen among women in this study.
    Conclusions: There seems to be a positive association between opioid use among men and incident cancer but not among women. However when we confine analysis to men taking opioids, an inverse relationship seems to be between increased opioid use and cancer. No evidence of connection between opioid use and cancer is to be seen among women. These preliminary results are somewhat contradictory and require further analysis.
