  • Algengi einhverfu hjá 7-9 ára börnum á Íslandi

  • Rannsóknir hafa sýnt að algengi einhverfu hefur farið jafnt og þétt hækkandi um allan heim frá því upp úr 1990. Síðasta íslenska rannsóknin sýndi algengið 1,2% en þá voru Greiningar- og ráðgjafarstöð ríkisins (GRR) og Barna- og unglingageðdeild Landspítalans (BUGL) einu stofnanirnar á Íslandi sem greindu börn með einhverfu. Markmið þessarar rannsóknar var að halda áfram að fylgjast með algengi einhverfu á Íslandi í tengslum við evrópskt samstarfsverkefni (ASDEU) byggt á aldurshópunum 7 til 9 ára. Að auki var skoðaður aldur við greiningu, niðurstöður þroskaprófa, tíðni þroskahömlunar og ýmsar breytur sem gætu verið mælikvarði á gæði greininga. Rannsóknin var aftursýn þversniðsrannsókn og var stuðst við upplýsingasöfnun úr sjúkraskrám þátttakenda. Þátttakendur voru 295 börn fædd á árunum 2006 til 2008 og voru greind með einhverfu á GRR eða BUGL fyrir árslok 2015. Niðurstöður sýndu að algengi allra raskana á einhverfurófi var um 2,2% og hefur því aukist verulega á skömmum tíma. Aukningin kemur fyrst og fremst frá drengjum sem greinast með bernskueinhverfu (ICD-10; F84.0) og með vitsmunaþroska nálægt meðallagi. Meirihluti barnanna (77,3%) fékk sína fyrstu greiningu á leikskólaaldri en samt voru mörg börn að greinast seint (eftir 6 ára) þrátt fyrir verulega skertan þroska. ADI-R greiningarviðtalið var notað mun sjaldnar á GRR en áður. Ýmsar skýringar á þessari aukningu á algengi einhverfu eru ræddar. Ályktað er um þörfina á að þróa nýja aðferðafræði við gagnasöfnun til að halda áfram að fylgjast með algengi einhverfu á Íslandi og mikilvægi klínískra leiðbeininga til að samræma greiningu á einhverfu á milli stofnana.

    Studies have shown that the prevalence of autism has steadily increased all over the world since the early 1990s. The latest Icelandic study showed a prevalence of 1.2% but at that time the State Diagnostic and Counselling Centre (SDCC) and the Department of Child and Adolescent Psychiatry (DCAP) were the only institutions which offered formal diagnostic services for children with suspected autism. The aim of this study was to continue to monitor the prevalence of autism in Iceland in relation to a European collaboration programme (ASDEU) based on the age groups 7-9-years. Age at diagnosis, results from cognitive development tests, frequency of intellectual disability and variables serving as indicators for the quality of diagnosis were also examined. A retrospective cross-sectional design was used and data were gathered by reviewing medical records. Participants were 295 children born in the years 2006 to 2008 and diagnosed with autism at SDCC or DCAP before the end of the year 2015. The results showed that the prevalence for all autism spectrum disorders was around 2.2% and has therefore increased considerably in a short period of time. The increase is mainly explained by boys diagnosed with childhood autism (ICD-10; F84.0) and relatively high functioning cognitively speaking. Majority of the children (77.3%) got their first diagnosis in the preschool years but still there were many children diagnosed late (after 6 years of age) despite significantly impaired cognitive development. The Autism diagnostic interview-revised (ADI-R) was used more rarely at SDCC than evident in previous studies. Several explanations for this increase in prevalence of autism are discussed. It is concluded that in light of changes in the service system, it is evident that there is a pressing need for a new approach to data gathering in order to continue to monitor autism prevalence in Iceland. These changes also indicate the importance of clinical guidelines to coordinate diagnosis of autism between service sectors and institutions.
