    Eftirlit sjúklinga hefur verið skilgreint sem samfellt ferli söfnunar, túlkunar og úrvinnslu upplýsinga sem nýttar eru til klínískrar ákvarðanatöku sem stuðlar að og viðheldur öryggi sjúklinga. Með auknum fjölda og veikari sjúklingum á legudeildum, ásamt breytingum í starfsemi sjúkradeilda, hefur dregið úr getu hjúkrunarfræðinga til að sinna fullnægjandi eftirliti sjúklinga. Það leiðir til þess að ekki er komið í veg fyrir atvik og öryggi sjúklinga er ógnað. Til að auka öryggi sjúklinga hefur á síðustu áratugum verið þróaður formlegur stuðningur sem ætlað er að styðja við starfsfólk deilda sem sinnir mikið veikum sjúklingum.
    Stuðningurinn er ýmist í formi gjörgæsluteymis, markvissrar fræðslu eða eftirgæslu sjúklinga sem útskrifast af gjörgæsludeildum. Allir þessir þættir hafa verið innleiddir á Landspítala í samræmi við stefnu um öryggi og fagmennsku. Meginmarkmið stuðningsins er að auka færni og þekkingu starfsfólks á deildum til að greina fyrr einkenni um versnandi ástand sjúklinga. Einnig að flytja sérhæfða þjónustu gjörgæsludeildar að rúmi sjúklingsins, veita honum meðferð við hæfi eða flytja á gjörgæsludeild ef þörf er á.
    Markmið þessa verkefnis var þróun mælitækis sem nota á við rannsókn á viðhorfi hjúkrunarfræðinga og deildarlækna til þess formlega stuðnings sem veittur er á Landspítala. Ekki fannst sambærilegt mælitæki við heimildaleit. Mælitækið er spurningalisti sem samanstendur af 39 spurningum um viðhorf og upplifun af störfum gjörgæsluteymis og eftirgæslu ásamt spurningum um öryggi sjúklinga og fræðslu starfsfólks. Mælitækið var rýnt af sjö sérfræðingum í hjúkrun ásamt rýnihóp hjúkrunarfræðinga og deildarlækna til að auka réttmæti þess. Mælitækið verður notað til rannsóknar á Landspítala en mikilvægt er að skoða árangur þess stuðnings sem innleiddur hefur verið, upplifun starfsfólks af honum og leita leiða til að auka öryggi sjúklinga.

  • Patient surveillance has been defined as a continuous process of acquisition, interpretation and synthesis of data utilized for clinical decision making to promote and maintain patient safety. With the increased number of patients and patient acuity on general wards, together with changes in the activities of the wards, the nurses’ ability to conduct adequate surveillance of patients has decreased. As a result, adverse events are not being prevented and patient safety is threatened. In recent decades, formal support to staff who take care of critically ill patients on general wards has been developed to increase patient safety.
    The support is either in the form of medical emergency teams, formal education or liaison nursing. This has all been implemented at Landspitali in line with its values of safety and professionalism. The main objective of the support is to increase the skills and expertise of ward staff so they are able to identify deteriorating patients. Also to transfer specialized intensive care services to the patient’s bed, give him appropriate treatment or transfer to the intensive care unit if necessary.
    The aim of this project was to develop a questionnaire which will be used for a study on the attitudes of nurses and residents to the formal support found in Landspitali. No comparable questionnaire could be found when searching reference material. The questionnaire contains 39 questions regarding the attitudes and experience of the medical emergency team and the liaison nursing, as well as questions about patient safety and staff education. The questionnaire was reviewed by seven clinical nurse specialists as well as a focus group of nurses and residents to increase validity. The questionnaire will be used in a study conducted in Landspitali as it is important to evaluate the results of the supportive measures implemented, the staffs’ experience and to find new ways to improve patient safety.
