    Faraldsfræði frárennslisvatns (e. sewage epidemiology) er nýleg og hagnýt aðferðafræði til þess að meta notkun fíkniefna og lyfja þar sem frárennslisvatns (FV) sýni endurspegla þvagsýni fyrir samfélgið í heild sinni. Hægt er að greina styrk efna í FV og bakreikna hann í neyslu lyfs á dag eða mánuði. Þannig er hægt að fylgjast með neyslumynstri ákveðinna lyfja.
    Markmið þessa verkefnis er að aðlaga og hámarka sýnameðhöndlunaraðferð fyrir morfín, kódein, oxýkódon, tramadól og O-desmetýltramadól (ODT) í frárennslisvatni. Neysla kódeins, morfíns, oxýkódons og tramadóls verður áætluð og borin saman við upplýsingar um útleysta lyfseðla frá Lyfjagagnagrunni Íslands.
    Fastfasa súluskiljun (e. solid phase extracton, SPE) er mikið notuð sýnameðhöndlunaraðferð fyrir FV sýni og var hún notuð í þessu verkefni. Hvert skref aðferðarinnar var hámarkað og svo var framkvæmd stutt gilding á henni. Sýni voru magnákvörðuð með ofur-háþrýstivökvagreini tengdum tvöföldum massaskynjara (UPLC-MS/MS). FV sýni yfir þriggja vikna tímabil árið 2016 voru greind. Sýnin voru fengin frá skólphreinsistöðvunum Sundaveitu við Klettagarða og Skerjafjarðarveitu við Ánanaust. Styrkur greiningarefna í FV var bakreiknaður í mg/dag/1000 íbúa og umbreytt í skilgreinda sólahringsskammta (e. defined daily dose, DDD) til þess að geta borið saman við gögn frá Lyfjagagnagrunni Íslands yfir útleysta lyfseðla frá árinu 2016.
    Vel gekk að þróa sýnameðhöndlunaraðferðina, heimtur aðferðarinnar voru góðar en áhrif annara efna (e. matrix effect) í FV voru til staðar fyrir greiningarefnin að ODT undanskildu. Greiningarefnin voru öll mælanleg í frárennslisvatninu. Neysla morfíns og oxýkódons var áætluð meiri en gögn Lyfjagagnagrunnsins gáfu til kynna en fyrir tramadól var neysla minni samkvæmt mælingum á frárennslisvatni heldur en samkvæmt útleystum lyfseðlum. Fullnægjandi gögn fyrir kódein yfir útleysta lyfseðla bárust ekki og því var ekki hægt að bera saman notkunina við bakreiknað magn.

  • Sewage epidemiology is a novel and practical methodology for assessing illicit and pharmaceutical drug use. The wastewater analysis can give an estimation of drug use in a specific community. It is possible to measure the drug concentration in wastewater and back calculate it to drug use per day or per month. This approach allows researchers to monitor drug consumption patterns more rapidly compared with currently used survey methods.
    The objectives of this research was to adapt and optimize a sample pre-treatment method for analysing morphine, codeine, oxycodone, tramadol and ODT in wastewater. Consumption estimation of morphine, codeine, oxycodone and tramadol were carried out and compared to prescription data from the Icelandic Medicines Agency.
    Solid phase extraction is a widely used sample pre-treatment method for wastewater samples and was therefore used in this study. Each step of the method was optimized and a short validation was carried out. Samples were quantitatively determined by using ultra-performance liquid chromatography joined with tandem mass spectrometry. Wastewater samples collected over three weeks in 2016 from wastewater treatment plants Sundaveita at Klettagarðar and Skerjafjarðarveita at Ánanaust were analysed. The concentrations of the analytes in the wastewater samples were then back calculated to mg/day/1000 inhabitants and further converted to defined daily doses for comparison with data from the Medicines Registry.
    The development of a sample pre-treatment was successful, recoveries for were within acceptable limits for all analytes but matrix effects were present for all analytes except ODT. All analytes were measurable in the wastewater samples. The estimated consumption from wastewater for morphine and oxycodone was higher than the data from the Medicines Registry. For tramadol there was less estimated consumption than the data from the Medicines Registry. Sufficient data regarding codeine prescriptions from the Medicines Agency was not accessible in time so comparison was not possible.
