  • Inngangur: Málstol er ein af mögulegum afleiðingum heilaskaða þegar skaði verður á málstöðvum heilans. Skaðinn hefur áhrif á málgetu einstaklingsins og eru einkennin mismunandi eftir staðsetningu skaðans en geta haft áhrif á máltjáningu, málskilning, lestur og ritun. Viðmælendaþjálfun tengd málstoli er fræðsla og þjálfun til aðila sem koma að samskiptum við fólk með málstol. Þjálfunin felur í sér fræðslu um eðli og einkenni málstols og þjálfun í samskiptastuðningi sem styður við samskipti við þennan hóp. Rannsóknir benda til að þekking á málstoli sé lítil á meðal almennings og heilbrigðisstarfsfólks sem getur útskýrt neikvæða upplifun fólks með málstol á samskiptum við aðra. Erlendar rannsóknir á viðmælendaþjálfun tengdri málstoli hafa sýnt fram á góðan árangur í bættum samskiptum við fólk með málstol, hvort sem um ræðir samskipti á milli aðstandenda og einstaklinga með málstol, sjálfboðaliða eða heilbrigðisstarfsfólks og fólks með málstol. Hérlendis hefur ekki verið gerð áður rannsókn á viðmælendaþjálfun tengdri málstoli.
    Markmið: Markmið þessa verkefnis var tvíþætt. Annars vegar að útbúa námskeið í viðmælendaþjálfun fyrir heilbrigðisstarfsfólk sem annast fólk með málstol út frá kenningum Supported Conversation for Adults with Aphasia (SCA) og Supporting Partners of People with Aphasia in Relationships and Conversation (SPPARC). Hins vegar að kanna reynslu af slíku námskeiði, þ.e. viðhorf starfsfólks til gagnsemi slíks námskeiðs, hvort það hafi haft áhrif á samskipti starfsfólks við fólk með málstol og hvernig þær aðferðir sem kenndar voru á námskeiðinu nýtast í starfi. Einnig hvaða þætti tengt málstoli og samskiptum starfsfólk telur sig þurfa að fræðast betur um eftir námskeiðið.
    Aðferð: Rannsóknin var eigindleg og var framkvæmt rýnihópaviðtal tveimur vikum eftir að námskeiðinu lauk og voru viðmælendur samtals sex heilbrigðisstarfsmenn sem kláruðu námskeiðið og höfðu reynslu af að vinna með fólki með málstol. Notuð var eigindleg efnisgreining við gagnagreininguna.
    Niðurstöður: Niðurstöðum rannsóknarinnar var skipt niður í tvö meginþemu. Annars vegar að samskipti við fólk með málstol eru krefjandi og hins vegar árangur viðmælendaþjálfunar. Þátttakendur lýstu samskiptum við fólk með málstol sem erfiðum og tímafrekum og upplifðu einnig óöryggi í samskiptum sem má rekja til skorts á upplýsingum og fræðslu um fólk með málstol. Eftir námskeiðið lýstu þátttakendur meiri skilningi og þekkingu á málstoli sem jók öryggi þeirra í samskiptum. Samskiptastuðningurinn sem kenndur var á námskeiðinu var talinn nýtast í starfi og töldu þátttakendur að námskeiðið væri gagnlegt fyrir annað heilbrigðisstarfsfólk, sérstaklega fyrir nýliða. Upplifun þátttakenda bendir til að viðmælendaþjálfun beri árangur og geti bætt aðgengi að samskiptum fyrir fólk með málstol, sem samræmist erlendum rannsóknum á viðmælendaþjálfun.

    Preface: Aphasia is caused by brain damage, as a result of a stroke, a tumor or a trauma, when damage occurs in the language areas of the brain. The symptoms depend on the location of the injury, and can affect language expression, comprehension, as well as the ability to read and write. Communication partner training is an educational and training program for people that interact with people who suffer from aphasia. The program includes education about aphasia and its symptoms, as well as training in communication support. Research indicate that knowledge about aphasia is lacking among people in the general public and among health care professionals. This might explain why people with aphasia report negative experience of communication with others. Research on communication partner training for professionals and relatives have shown improved communication with people with aphasia, both when looking at communication between relatives and individuals with aphasia, and communication between volunteers or health care professionals and people with aphasia. Communication partner training for aphasia has not been investigated in Iceland before.
    Aim: The aim of this project was twofold. First, to prepare a course in communication partner training for health care personnel helping people with aphasia, derived from the theories of Supported Conversation for Adults with Aphasia (SCA) and Supporting Partners of People with Aphasia in Relationships and Conversation (SPPARC). Second, explore the views of professionals on possible benefits of such a course, whether it had beneficial effect on communication with people with aphasia and if the methods taught in the course were useful and practical in their work. Also, what aspects related to aphasia and communication, professionals feel they need to learn more about after the course.
    Methods: The research design of this study was qualitative with a focus group interview conducted two weeks after the end of a communication partner training course. The interviewees were a total of six health care professionals who completed the course and had experience of working with people with aphasia. Qualitative content analysis was used in the data analysis.
    Results: The results of the study were divided into two main themes. On the one hand, that communication with people with aphasia is demanding and, on the other hand, the effectiveness of communication partner training. Participants described communication with people with aphasia being difficult and time-consuming and that they experienced insecurity in communication that could be attributed to a lack of information and education about people with aphasia. After the course, participants expressed a greater understanding and knowledge of aphasia that increased their communication skills. The communication support taught during the course was considered useful for health care personnel, especially for those new in the field. The participants‘ experience indicates that communication partner training is useful and can improve access to communication for people with aphasia, which is in line with previous research on communication partner training.
