  • Inngangur: Notkun ómskoðunar (e. diagnostic ultrasound) hefur verið að ryðja sér til rúms meðal sjúkraþjálfara síðustu ár en minna hefur borið á notkun tækjanna hér á landi. Helstu ástæður þess geta verið mikill kostnaður, tímaleysi og kröfur um mikla sérhæfingu. Í gegnum tíðina hafa fáar rannsóknir verið gerðar sem sýna fram á notagildi ómskoðunartækja fyrir sjúkraþjálfara en í nýlegum rannsóknum hefur þetta viðfangsefni verið skoðað.
    Markmið: Markmið rannsóknarinnar er að gera „scoping” samantektarrannsókn um notagildi ómskoðunartækja við skoðun og meðferð sjúkraþjálfara í daglegu starfi. Gerð verður grein fyrir kostum og göllum er varða notkun ómskoðunartækisins og reynt að komast að því hvort sjúkraþjálfarar ættu að nýta sér ómskoðunartæki í daglegu starfi. Þetta er fyrsta „scoping“ samantektarrannsóknin sem rituð er á íslensku um notagildi ómskoðunartækja fyrir sjúkraþjálfara. Leitast var við að svara eftirfarandi spurningu: Ættu sjúkraþjálfarar að nýta sér ómskoðunartæki við greiningu og meðferð í daglegu starfi?
    Aðferð: Leitað var í gagnagrunnum Medline – Pubmed, Pedro og Science direct – Elsevier að rannsóknum sem lýstu notagildi ómskoðunartækja í daglegu starfi sjúkraþjálfara. Rannsóknir voru á ensku eða íslensku og birtar eftir 1. janúar 2011. Rannsóknir voru af sönnunarstigi I-III. Útilokaðar voru rannsóknir sem skoða ekki mögulegt klínískt notagildi ómskoðunartækja fyrir sjúkraþjálfara.
    Niðurstöður: Við leitina komu upp 428 rannsóknir en fjórar þeirra uppfylltu inntökuskilyrði „scoping“ samantektarrannsóknar. Ein rannsókn fannst auk þess við skoðun heimildalista. Þrjár rannsóknir voru kerfisbundnar samantektarrannsóknir, ein rannsókn var framskyggn ferilrannsókn og ein rannsókn var samtíma réttmætis og milli matsmanna áreiðanleikarannsókn. Niðurstöður rannsókna sem uppfylltu inntökuskilyrði sýndu jákvæðar vísbendingar um notagildi ómskoðunartækja fyrir sjúkraþjálfara. Erfitt er þó að meta klínískt notagildi ómskoðunartækja fyrir sjúkraþjálfara vegna mismunandi aðferða við mælingu og notkun ómskoðunartækja auk fjölbreytilegrar uppbyggingar sina og vöðva sem mæld eru í rannsóknum. Áreiðanleiki milli matsmanna og innri matsmanns á mælingum með ómskoðunartæki gefa vísbendingar um fínar líkur á áreiðanleika niðurstaða mælinga með ómskoðunartækjum.
    Samantekt: Fáar hágæða rannsóknir um notagildi ómskoðunartækja fyrir sjúkraþjálfara fundust við leitina. Jákvæðar vísbendingar um notagildi ómskoðunartækja fyrir sjúkraþjálfara geta leitt til þess að notkun á ómskoðunartækjum fyrir sjúkraþjálfara aukist töluvert á komandi árum. Frekari hágæða rannsókna er þörf til að álykta betur um klínískt notagildi ómskoðunartækja fyrir sjúkraþjálfara.

    Introduction: The use of diagnostic ultrasound has been increasing amongst physiotherapists in the past few years, although it has not gained momentum or implementation in Iceland. The main reasons for that could be the cost, time constraints and the specialization needed. Through the years there are few researches that have focused on the benefits of diagnostic ultrasound for physiotherapists, although recent studies have focused on the subject.
    Aim: The aim of this study is a scoping review regarding the usability for physiotherapist in implementing diagnostic ultrasound for their daily diagnosis and treatment. The advantages and the disadvantages have been compared as to the use of diagnostic ultrasound with the aim to find out if physiotherapists should implement diagnostic ultrasound in their standard practice. This scoping study is the first summary study to be written in Icelandic regarding the advantages and usability of diagnostic ultrasound for physiotherapists. This comparative study seeks to answers the question; Should physiotherapists implement diagnostic ultrasound equipment during diagnosis and treatment in their daily practice?
    Method: The databases of Medline - Pubmed, Pedro and Science Direct – Elsevier were searched and viewed for any research describing the usability of using diagnostic ultrasound for physiotherapists during daily practice. Search parameters consisted on research papers in English and Icelandic with publishing dates dating back as far as the 1st of January 2011. These researches had level of evidence of I-III. Researches not focusing on the clinical usability of diagnostic ultrasound for physiotherapists were excluded during this process.
    Results: The search criteria yielded a total of 428 results, with a total of five that met the search criteria of scoping summary study. A total of three of these were systematic summary studies, one was a research format prospective cohort study and finally one research was concurrent validity and inter-rater reliability study. The conclusion of these five research studies shows positive indications regarding the advantages/usability of implementing diagnostic ultrasound equipment in physiotherapists daily practice. Although it is not without considerable difficulties to clinically prove the usability for physiotherapists due to the various methods and parameters of measurements of ultrasound equipment. Also due to the structural variability of tendons and muscles that were measured. The reliability and consistency between the assessors and internal assessors on the measurement parameters of the diagnostic ultrasound for physiotherapists that were discovered during the data gathering phase, indicate positive probabilities as to the reliability of the measurements regarding the diagnostic ultrasound device.
    Conclusion: There is a limited amount of published quality research studies that focus merely on the advantages of using diagnostic ultrasound for physiotherapists. There are some positive indications that there are some advantages for physiotherapists and the usability of diagnostic ultrasounds could increase considerably in the near future. Further quality research studies are required to conclude on the clinical usability of implementing diagnostic ultrasound for physiotherapists.
