Niðurstöður: Samtals voru 57.543 lyfjalistar yfirfarnir fyrir 25.146 einstaklinga.

  • Introduction: Multimorbidity is becoming more prevalent as the nation ages and thus also polypharmacy. Many events can occur in medicine including prescribing cascade, poor medication adherence, adverse events and, drug interactions. Research shows that with regular medication reviews, many problems related to medications can be prevented. Medication reviews have been proven useful for different patients in different levels of health care. The objective of this research was to look at the group who received a medication review with regard to various factors broken down by gender and age.
Methods: The aim of this retrospective cohort study was to process data, interpret it and describe it. The study population was all patients 60 years and older in primary care in the Reykjavík metropolitan area who had their medication list reviewed. The study period was from the beginning of 2017 through September 2021. The data was described by prevalence figures and proportions.
Results: 57.543 medication lists were reviewed for 25.146 different patients. Women were in a majority in most subgroups and received more reviews on average. Different types of communication were used but interviews were most prevalent. A majority of those who received a medication review were subject to polypharmacy and it became more prevalent with higher age. Almost 20% of the population were dose dispensing patients at some point of the study. The youngest subject was 60 years of age and the oldest 105. Many of the most common medications were in the first level ATC group N and the most common fourth level ATC group was proton pump inhibitors.
Conclusions: The findings show that the number of clients of primary care in the Reykjavík metropolitan area who received a medication review increased significantly. Most reviews were conducted in an interview and polypharmacy was prevalent.
