    Obstructive sleep apnea (OSA) is a common condition associated with mental disorders and cognitive dysfunction. The aim of the study was to investigate symptoms of depression, anxiety, and stress, along with emotion processing among individuals with and without OSA diagnosis, including differences in OSA severity. Participants were 60 individuals aged 20-80 years (51.6% female, 48.4% male), divided into four groups based on overnight polysomnography recordings: healthy controls, mild OSA, moderate OSA, and severe OSA. Participants completed the Depression, Anxiety, and Stress Scale-21 (DASS-21) and the Penn Emotion Recognition Test (ER-40). Analyses revealed significantly higher anxiety symptom scores in the severe OSA group compared to healthy controls and the mild OSA group. There were emotion processing deficits in total accuracy among OSA patients, with the severe OSA group less accurate than the control and moderate OSA groups. Further analysis for low-intensity emotional stimuli revealed significant group differences, with the severe OSA group less accurate than the moderate OSA group and slower than the mild OSA group. Accuracy and reaction time (RT) analyses for high-intensity emotional stimuli did not reveal significant differences or interactions. These findings suggest that OSA severity plays a role in mental health and emotion processing, with worse mental health and emotion processing deficits in more severe OSA patients. Future studies should investigate differences in OSA severity in mental health and emotion processing.
