Fjölþættur vandi og þjónustukerfin á Íslandi: Upplifun og reynsla ungmenna og fagfólks af þjónustu innan félags-, mennta og heilbrigðiskerfa

Abstract

Rannsóknin fjallar um þjónustu við ungmenni með fjölþættan vanda, þar sem samverkandi vandi og margþættar áskoranir kalla á samþætta og sérhæfða þjónustu frá unga aldri. Þrátt fyrir að hugtakið fjölþættur vandi sé ekki formlega skilgreint í lögum er það notað um börn með alvarlegar þroska- og/eða geðraskanir og hegðunarvanda sem hafa veruleg áhrif á daglegt líf. Þrátt fyrir lagalegan rétt barna til verndar, þátttöku og viðeigandi þjónustu benda rannsóknir og úttektir til þess að þjónustuflæði sé ósamfellt og verkferlar óskýrir. Markmið rannsóknarinnar var að varpa ljósi á reynslu ungmenna sem hafa lifað með fjölþættum vanda og komið í snertingu við mennta-, heilbrigðis- og velferðarkerfi, auk þess að skoða sýn fagfólks með langa reynslu af vinnu með þeim börnum og ungmennum. Rannsóknin byggir á eigindlegri, fyrirbærafræðilegri og túlkunarfræðilegri nálgun. Tekin voru viðtöl við tvo hópa, annarsvegar fimm viðtöl við einstaklinga sem höfðu sem ungmenni reynslu af þjónustunni, og hinsvegar við þrjá úr hópi fagfólks. Niðurstöður sýna að erfiðlega gengur að afmarka hugtakið innan lagaramma og að mörg ungmenni falli á milli kerfa. Þjónustan reynist oft ósamfelld og ómarkviss, með takmörkuðu upplýsingaflæði og samvinnu. Ungmenni lýstu vantrausti, löngum biðtímum og litlu svigrúmi til áhrifa á eigin mál, á meðan fagfólk benti á kerfislegar hindranir, skort á úrræðum og þörf fyrir aukna samþættingu og teymisvinnu.Rannsóknin undirstrikar mikilvægi snemmtækrar íhlutunar, virkrar þátttöku barna og mannlegrar, heildrænnar nálgunar. Niðurstöðurnar geta haft hagnýtt gildi fyrir stefnumótun og þróun þjónustu. This study examines services provided to young people with complex needs, where interacting challenges require coordinated, specialized, and integrated support from an early age. Although the term complex needs is not formally defined in legislation, it is commonly used to describe children with severe developmental and mental health disorders combined with behavioural difficulties that significantly affect their daily functioning. The number of children in this group has increased rapidly, from an estimated 8–12 in 2013 to approximately 130 today, reflecting a growing need for effective and individualized services. Despite children’s legal rights to protection, participation, and appropriate support, research indicates that service pathways are often fragmented and procedures unclear. The aim of this study was to explore the experiences of young people who have grown up with complex needs and interacted with educational, health, and welfare systems, as well as to examine the perspectives of professionals with extensive experience working with this group. The study is based on a qualitative, phenomenological, and interpretive approach. Interviews were conducted with five young people with lived experience of services, and with three professionals working with children and youth with complex needs. The findings show that the concept is difficult to delineate within current legal frameworks and that many young people fall between systems. Services often appear inconsistent and uncoordinated, with limited information flow and collaboration. Young participants described mistrust, long waiting times, and restricted influence over their own cases, while professionals highlighted systemic barriers, lack of resources, and the need for increased coordination and interdisciplinary teamwork. The study underscores the importance of early intervention, active participation of children, and a humane, holistic approach. The findings have practical implications for policy development and service improvements, emphasizing the value of incorporating the voices of both service users and professionals in creating empowering and integrated support for young people with complex needs.Keywords: complex needs, youth, social services, professionals, integration
